Patient Registrations

To register with the practice you must be living within the practice catchment area within Wandsworth or Merton. 


  1. You can register by coming into the practice to complete a registration form below.

  2. All newly registered patients will be required to complete a medical information questionnaire issued by the Practice. (You will need to complete this full form all 6 pages and bring it into the practice to complete the registration process).

If you are transferring from another Practice and have your NHS Medical Card or the details of your previous Practice, then you will only be required to have: 

  • One proof of address for ID (utility bill dated within the last 3 months)


If you do not have your NHS Medical Card or are registering for the first time, you will also need to bring your:

  • Passport

  • Driving license

  • Proof of address (utility bill dated within the past 3 months, council tax statement or tenancy agreement)

Registration Regulations

Passport or Driving Licence, Proof of address i.e utility bill (dated within the past 3 months) council tax statement or tenancy agreement.

Patients from EU

Passport or Driving Licence, Proof of address i.e utility bill (dated within the past 3 months) council tax statement or tenancy agreement.

Patients from abroad

Passport or Driving Licence, Proof of address i.e utility bill (dated within the past 3 months) council tax statement or tenancy agreement and proof of residency.

Temporary Registration

The practice accepts temporary registered patients for immediate necessary treatment. To register temporarily you will need to complete a registration form. Please note if you are an overseas visitor you will be entitled to immediate necessary treatment only under NHS.

Book an Interpreter

If English is not your first language then we can book an interpreter for your appointment. Please speak to reception as to how to arrange this before you book your appointment.


Change of address

If you have changed address please let us know. You will also need to complete a change of details form and hand it into reception in order for us to update our records. Our reception team will also check your new address to see if it falls within the practice area. If you have moved out of the area then you will need to register with a GP closer to your new address.
